In the underground, miners who fake an illness are scolded and thrown out of the clinic by the doctor — not because the doctor lacks empathy, but because her time is too precious and must be reserved for those who really need medical treatment.
After the Blockade was ordered, very few clinics remained open in the Underworld. This one is the only remaining medical facility in the mining town — though it may not last much longer. Medical instruments and drugs are gradually being used up each day, which has the clinic's doctor Natasha worried. The Fragmentum and the monsters bring more and more patients, and a lively clinic is not something a doctor wants to see.
In the game and dialogue, the Clinic is referred to as "Natasha's Clinic," but the sign on the building reads "Vache's Clinic." This suggests that the clinic may have been previously owned by Natasha's adoptive brother, Vache.
